How do I make a claim?

If you need to make a claim, please complete a Claim Form and return it to us as soon as possible. To speed up the process you can download a Claim Form rather than waiting for a claim form from us by mail.

I have lost my Product Disclosure Statement (PDS).

If you require a copy of our Product Disclosure Statement please contact us.

How can I have an endorsement removed from my policy?

To apply for the removal of an endorsement, after the end of your recovery periods, please forward us signed and dated notification from a licensed medical practitioner, stating the period of time that you have been symptom-free of your sickness condition/s.

Are my children covered under this policy?

Eligible children can be covered under certain Personal Insurance: Your Choice Plans. Contact Combined Insurance for more information.

Am I covered on holiday?

Depending on which country you are on holiday in and the type of cover you hold.  Please see policy wording in the Policy Disclosure Booklet for full cover details. If you require a copy of the Product Disclosure Statement please contact us.

I’ve lost my job - can I keep my policy?
